Medical scheme administration, risk management, and technology cluster

Our collaboration with Sanlam is progressing well, with the first phase successfully implemented to support their partnership with Fedhealth. As part of our commitment to incentivising wellness and proactive healthcare behaviours, we have co-designed a rewards programme with Sanlam, set to launch in 2025. This initiative encourages members to actively engage in preventative health measures and wellness activities, leading to better health outcomes and stronger scheme retention rates.

To create a seamless healthcare experience, we have prioritised integrating pharmaceutical and administrative services, ensuring members can access care more efficiently across various health touchpoints. Medscheme, our healthcare administration and health risk management business, is also actively strengthening its client value proposition by exploring opportunities to cross-sell insurance products and co-develop value-driven health solutions with Sanlam. These efforts drive product innovation, reduce costs, and improve healthcare accessibility, reinforcing our commitment to delivering affordable, high-quality healthcare solutions for members.

In line with our focus on enhancing affordability, we successfully implemented specialist contracting in partnership with Medshield, ensuring members have access to a network of over 700 specialists. This initiative enhances affordability by negotiating competitive reimbursement rates while maintaining high-quality care standards. By streamlining specialist referrals and improving care coordination, we drive cost efficiencies and optimise patient outcomes across our administered schemes.

We remain dedicated to expanding healthcare access by reducing costs and financial barriers. To achieve this, we have developed cost-effective bundled products that lower entry-point fees, making healthcare more accessible to a broader population.

Maintaining exceptional service standards is a top priority as we continue to enhance our healthcare offerings. We are strengthening partnerships with medical schemes, intermediaries, and healthcare providers to improve member experiences and drive retention. Our commitment to excellence has been widely recognised. In December 2024, Medscheme received a Long Service Award from the Government Employees Medical Scheme (GEMS) at their prestigious gala event, underscoring our longstanding contribution to healthcare administration and risk management. With a 5.24% increase in total lives covered from July 2023 to December 2024, AfroCentric continues to expand its impact, reinforcing its role as a leader in healthcare solutions.

Pharmaceutical cluster

Pharmacy Direct plays a critical role in building a winning health offering by driving affordability through the increased use of generic medicines, which offer the same efficacy and safety as originators at a significantly lower cost. Over the past three years, this initiative has contributed to a steady decline in the average cost of chronic prescriptions. Additionally, Scriptpharm’s network pharmacy adherence model has achieved a 90%+ generic substitution rate, maximising the value of medical benefits for members. Additionally, we have adopted capitation-based contracting models, resulting in significant cost savings. For example, over the last five years, R31 million medicine cost savings were achieved for one of our large, contracted open schemes through Scriptpharm’s cost-containment strategies.

Our ongoing partnership with the DoH through the CCMDD programme has reinforced our role as a trusted provider for public healthcare. By leveraging our expertise in large-scale medicine distribution, we are well-positioned to collaborate with both the public and private sectors to advance the objectives of the NHI.

Corporate solutions cluster

A robust healthcare system extends beyond traditional medical schemes, encompassing comprehensive employee health and wellness solutions that prioritise prevention and early intervention. By focusing on proactive healthcare measures, we improve health outcomes, reduce long-term healthcare costs, and enhance overall workplace productivity.

To support early detection and preventative care, we offer on-site, nurse-led primary care clinics that provide wellness screenings, immunisations, treatment for mild illnesses, and general medical advice. These clinics ensure that employees have immediate access to essential healthcare services without needing to take extended time off work, ultimately reducing absenteeism and improving productivity.

Through strategic partnerships, we are expanding affordable, flexible healthcare options. The Fedhealth Elect and Savvy medical aid options, when combined with Fedhealth NexGen Gap Cover by Sanlam and Sanlam Primary Care health insurance, provide comprehensive cover at a lower cost, offering members protection from penalty co-payments and increasing access to enhanced out-of-hospital benefits. Additionally, our automated claims process for Sanlam Gap Cover clients – integrated with medical schemes administered by Medscheme – has improved efficiency, reduced processing costs, and enhanced client experience.

By prioritising workplace wellness, integrated healthcare solutions, and process innovation, we are ensuring that employees receive accessible, cost-effective, and high-quality care, supporting both their health and business sustainability.

Building a sustainable, integrated, patient-focused cancer care model

Cancer is more than a diagnosis – it is a journey filled with uncertainty. But with the right support, it can also be one of hope and resilience. AfroCentric is committed to ensuring cancer patients receive the care they need without overwhelming financial burdens. Through cost-containment measures, performance-based reimbursement models, technological innovation, and specialist network expansion, we are making high-quality cancer treatment more accessible and affordable.

Lowering costs and ensuring financial stability

One of our key priorities is making cancer treatment more affordable. At Scriptpharm, over the past four years, we have maintained lower dispensing fees for all out-of-hospital cancer medications and consumables, ensuring that treatment remains within reach. Our approach has also helped stabilise medical scheme costs, keeping year-on-year expenses below inflation for most schemes. Our cost-saving initiatives have resulted in R23 million in savings for three of the largest medical schemes we support.

Aligning treatment costs with real-world outcomes

To ensure that members receive high-quality, cost-effective cancer care, we have implemented a performance-based reimbursement model. This model directly links treatment costs to measurable patient outcomes, driving affordability and effectiveness. Additionally, our anti-emetic funding policy guarantees that supportive medications – critical for managing chemotherapy side effects – are available at the most cost-effective price. By collaborating with a network of providers, we have created a preferred product list, ensuring standardised, high-quality treatment options while negotiating better pricing for patients and schemes.

We have developed a comprehensive data model to track and optimise cancer treatment delivery, ensuring greater efficiency and better patient outcomes. Our advanced pricing analysis systems also continuously assess medicine treatment costs, identifying affordable alternatives without compromising quality. Historically, healthcare systems have been structured around a fee-for-service model, where healthcare providers are paid based on the number of services they deliver rather than the quality or effectiveness of those services. At AfroCentric, we are challenging this approach by shifting the focus to patient outcomes, ensuring that the care provided leads to meaningful improvements in health while also reducing unnecessary costs.

A core pillar of our strategy is therefore value-based care, which shifts the focus from volume-driven healthcare to improved patient outcomes, cost efficiency, and quality care. This model ensures that members receive holistic, coordinated, cost-effective treatment.

To drive this transformation, we have begun implementing several value-based care initiatives, including:

  • The Orthocare Spinal Risk Transfer Model, developed in 2024 and launched on 1 January 2025 for selected schemes, is a key initiative in transforming traditional care delivery into a value-based model. The programme implements a multi-disciplinary biopsychosocial rehabilitation approach, integrating psychological support and exercise therapy to enhance pain management, improve function, and develop coping skills. This is achieved through the documentation-based care process, which ensures a structured, evidence-based treatment pathway. By prioritising conservative management strategies, the initiative aims to enhance clinical outcomes for patients with chronic low back pain while reducing unnecessary spinal surgeries and interventions, ultimately lowering long-term healthcare costs
  • The Oncology Performance-Based Reimbursement Model, which lays the groundwork for evidence-based oncology care, ensuring optimal treatment pathways while managing costs
  • Integrated Pharmacy Benefit Management, which enhances medicine cost-effectiveness by introducing formulary revisions, AI-driven screening benefits for chronic conditions, and expanded oncology networks

AfroCentric is reshaping this approach through value-based pharmaceutical contracting, which aligns medication costs with real-world treatment effectiveness. As we advance our managed care approach, we are expanding specialist networks, optimising provider contracting models, and integrating cutting-edge care management solutions. By focusing on prevention, early intervention, and efficient care pathways, we are improving patient outcomes while ensuring the sustainability of healthcare funding.

As part of our 2024 managed care initiatives, we enhanced cost efficiency, early intervention, and personalised treatment pathways to drive better health outcomes across all administered schemes:

  • Optimising medicine formularies to ensure cost-effective treatment without compromising access to essential medications
  • Implementing AI-driven diabetes screening to enable earlier detection and intervention, helping to reduce long-term healthcare costs and complications
  • Launching a new obesity care plan, addressing the rising impact of lifestyle-related conditions with tailored, preventative interventions
  • Expanding our oncology network to provide more cost-effective treatment pathways, ensuring that patients receive high-quality cancer care with optimised affordability
  • Introducing enhanced case management criteria, successfully reducing hospital length of stay, leading to better resource utilisation and improved patient recovery times

A healthy workforce is a productive workforce. We recognise that employee wellbeing extends beyond medical care to include preventative health measures, mental wellness, and financial stability. By offering tailored corporate healthcare solutions, we empower businesses to take a proactive role in supporting their employees’ long-term health and productivity.

As people age, early detection of health risks is essential for preventing severe illnesses and maintaining a good quality of life. To support this, AfroCentric Wellness, a business within the Corporate Solutions cluster, has launched a pilot programme focused on wellness screening and health incident prevention. This initiative enables the early identification of age-related health conditions, allowing for timely medical interventions that help members stay healthier for longer while reducing the need for costly emergency treatments. As a key enabler of value-based care, this pilot not only enhances preventative healthcare but also has the potential to expand into broader primary care solutions, further strengthening AfroCentric’s commitment to delivering efficient, proactive, and patient-centred healthcare.

Mental health is an essential but often overlooked aspect of workplace wellbeing. In 2024, we launched a cost-effective employee wellness programme to make mental health support more accessible. This initiative provides employees with access to mental health professionals, digital wellness tools, and preventative resources – all at a fraction of the cost of traditional employee assistance programmes. By making mental health care more affordable and accessible, we are helping businesses create healthier, more resilient workforces.

Financial insecurity is one of the leading causes of stress, significantly impacting both mental and physical health. Understanding this link, we have developed financial wellbeing solutions to help employees manage debt, plan for savings, and improve financial stability. By reducing financial stress, employees can focus on their work and wellbeing, leading to higher productivity, better engagement, and improved long-term health outcomes.

To further improve workplace healthcare access, we have partnered with Sanlam Life to introduce a mobile nurse service. This initiative enables nurses to visit clients at their preferred location to collect blood specimens for underwriting purposes, eliminating the need for travel and streamlining the insurance application process.

By integrating preventative healthcare, mental wellness, and financial stability initiatives, we are helping businesses create healthier, more engaged, and more productive employees. Our commitment to proactive and accessible healthcare solutions ensures that employee wellbeing remains at the heart of a thriving workplace.

Clinical innovation: Advancing research to improve healthcare outcomes

We are committed to enhancing healthcare quality through research-driven, evidence-based managed care. Our longstanding collaboration with IeDEA Southern Africa (IeDEA-SA), a global HIV research consortium, allows us to contribute valuable health data that informs clinical decision-making, intervention strategies, and public health policies. By leveraging insights from this partnership, we are driving improvements in HIV treatment, mental health support, and cancer prevention – advancing care for members and beyond.

Leveraging data for better patient outcomes

Medscheme has contributed to IeDEA-SA’s research for many years, sharing Aid for Aids programme data and client-approved datasets covering members with and without HIV. Initially focused on antiretroviral therapy (ART) outcomes, recent research has expanded to non-communicable diseases and mental health.

Groundbreaking research findings

Three IeDEA-SA studies have been published in peer-reviewed journals, offering critical insights into mental health, mortality risks, and cancer prevalence among members. These findings reinforce the need for targeted interventions to improve long-term health outcomes:

  • Mental health and HIV outcomes: A study of 54 378 individuals found that those with mental health diagnoses had lower adherence to HIV treatment and reduced viral suppression. This highlights the urgent need for mental health support within HIV care programmes, particularly for adolescents and men.
  • Mental illness and premature mortality: Tracking over 1 million beneficiaries for three years, this study revealed significant excess mortality among those with mental illnesses, primarily from natural causes. The highest loss of life expectancy was among men with substance use disorders. These findings underscore the need for integrated mental and physical health interventions.
  • Cervical cancer risk among women with HIV: In a study of 517 312 women, cervical cancer rates were found to be three times higher in women with HIV than in those without. The study also found that cervical cancer develops earlier in women with HIV. This evidence highlights the importance of prevention, early detection, and treatment strategies, particularly for young and middle-aged women.

By integrating mental health support into HIV programmes, strengthening preventative care for high-risk populations, and refining value-based reimbursement models, we continue to ensure that our healthcare interventions are informed by real-world data.

Through collaborative research, data-driven innovation, and evidence-based policy development, we are shaping a more proactive, patient-centred healthcare system – one that delivers improved health outcomes and long-term sustainability for members and communities.

Investing in data, digital, and technology

The healthcare industry is undergoing a digital transformation. AfroCentric is at the forefront of integrating data analytics, automation, and emerging technologies to improve service delivery, enhance member experiences, and drive operational efficiency. By investing in advanced claims processing, robotic process automation (RPA), digital servicing platforms, and predictive analytics, we are creating a more responsive, cost-effective, and patient-centred healthcare ecosystem.

Medical scheme administration, risk management, and technology cluster

Our medical scheme administration, risk management, business enablement and technology clusters have been on a collaborative modernisation journey to replace legacy claims modules with a modern architecture that is quick and easy to configure, is able to accommodate complex claims rules, is scalable and able to process claims in real-time. The cluster processes an average of 10.5 million associated provider claims per month with an automation rate exceeding 99%, significantly improving speed and accuracy in claims processing. Similarly, hospital and facility claims – previously reliant on legacy systems – now achieve an automation rate of 68%, an increase of 20% compared to the legacy claims system. Our decision services technology supports hospital pre-authorisation and case management automation, processing approximately 260 000 facility case transactions monthly. Of these, over 100 000 are concluded by the decision engine, an increase of approximately 12 000 per month compared to the previous year, enhancing automation and streamlining patient access to hospital care.

As part of our strategic investment in data, digital, and technology, we continue to harness automation opportunities to drive efficiency, accuracy, and scalability across critical processes. Our use of hyper-automation, including RPA, has significantly enhanced our ability to manage large-scale system updates while reducing operational burden. We have expanded our process automation to include 57 business processes, processing an average of 153 000 work items per month, utilising 100 bots with an automation completion rate above 95%. These enhancements are not just about improving efficiency; they are part of our broader strategy to build a digitally enabled, future-fit organisation that delivers greater agility, resilience, and customer-centricity.

During our year-end processes, automation played a pivotal role in updating medical scheme limits and streamlining testing. We deployed 14 new automated processes for year-end optimisation, while existing ones maintained a completion rate of over 95%, cutting the total effort required from 350 days to 111 – more than three times faster. RPA delivered the most significant gains, reducing manual processing from 236 days to just over 6, while test script automation improved from 114 days to 105. These efficiencies enabled us to meet all deadlines despite late change requests, ensuring seamless continuity in customer service.

Our digital transformation strategy extends to customer interactions, with the implementation of speech analytics in all Medscheme contact centres. This technology monitors customer sentiment and agent behaviour, providing real-time insights that help managers address service failures and improve the quality of interactions. Plans are in place to extend this technology across email and digital servicing channels in all official languages, ensuring enhanced customer service monitoring across all our servicing channels and the ability to improve customer service experience.

To empower healthcare providers, we have enhanced self-service capabilities through our provider portal, launched in July 2024. More than 15 000 medical practices and 200 bureaus now use the platform to review claims, validate member benefits, access funding documents, and process payment remittances. A dedicated engagement team has been established to support further uptake. Our member experience strategy has also embraced innovative digital tools, including the rollout of quick response (QR) codes at medical practices. Piloted in August 2024, the QR code functionality has expanded to 1 500 practices, with 400 actively using it. The tool allows members to access practice information, check their benefits, nominate a preferred provider, and submit specialist referrals. This initiative is being actively promoted at healthcare facilities by Provider Liaison Consultants, ensuring greater adoption.

We recognise that while technology adoption has been a focus, constraints in cloud infrastructure and fragmented customer service records have made it challenging to deliver a seamless experience. To address this, we launched a digital roadmap in 2025, which includes modernising customer engagement architecture to create a single, consolidated, omnichannel view of each member’s journey. This will allow for more personalised interactions and proactive service delivery. Enterprise architecture is a key component of our technology investment, with dedicated solution architects now in place to ensure alignment with AfroCentric’s broader strategic vision. Investing in the right digital skills and leadership has been critical in ensuring sustainable innovation across our healthcare ecosystem.

A key achievement in our data management modernisation has been the migration of procedure files to a new, centralised data management solution – laying the foundation for greater accuracy, efficiency, and scalability. Automation of business decisions relies on master and reference data. Mastering industry data is crucial for our success. We have developed a single Enterprise Data Exchange where industry data is centrally managed and made available for utilisation to all our applications, including the decision engine. Previously, data was mastered in each application; now, we can create one version of the truth, reducing the risk of data errors and the limited resource capacity required to maintain the data.

Looking ahead, our digital roadmap and enterprise architecture strategy will continue to drive innovation and integration across our healthcare ecosystem. By overcoming legacy system challenges, investing in cloud-based infrastructure, and refining customer engagement through AI-powered tools, we are building a future-ready healthcare platform that benefits members, healthcare providers, and corporate clients.

Pharmaceutical cluster

Pharmaceutical care is another area where data-driven insights and automation are improving efficiency. At Pharmacy Direct, we have implemented e-scripting technology to streamline prescription processing, reducing the administrative workload for healthcare providers. Similarly, Scriptpharm’s oncology medicine data model is driving better cost management and treatment planning, with automated, rule-based applications for anti-emetic drug authorisations ensuring that members receive the right medication at the most cost-effective price (please see our case study for a more comprehensive view of how we are optimising oncology care).

To support our digital shift, we are actively working with software partners to implement customised dispensing solutions that align with South Africa’s unique healthcare funding environment, which includes over 70 medical schemes and 600 benefit options. International solutions are often not tailored to these complexities, making local innovation essential. Through this initiative, we are ensuring that pharmaceutical claims processing remains efficient, scalable, and adaptable to evolving healthcare needs.

As we expand our digital capabilities beyond individual member support, we are leveraging emerging technologies to create more personalised, data-driven, and scalable corporate wellness solutions. By integrating AI, automation, and digital engagement tools, we strengthen workplace wellbeing strategies and enable employers to foster healthier, more productive work environments.

One of our key innovations is the Staff Lifestyle App, which enhances corporate wellness by providing AI-driven push notifications that deliver personalised wellness support, timely reminders, and self-help tools. This confidential, user-friendly mobile platform ensures that employees have access to support tailored to their individual needs anytime, anywhere. Employers also benefit from anonymised data-driven insights, allowing them to assess wellness programme effectiveness, optimise resource allocation, and proactively address workplace wellbeing. These capabilities contribute to improved engagement, reduced absenteeism, and increased productivity, helping businesses create more resilient workforces.

While digital wellness solutions offer significant benefits, participation has been hindered by two key challenges: a lack of understanding of the technology scope among clients and low adoption rates. To address these barriers, we have implemented targeted customer education and engagement strategies, including gamification, incentives, and launch campaigns. These initiatives have proven effective in increasing awareness and participation, ensuring employers and employees fully engage with and benefit from digital wellness solutions.

Our medical scheme administration, risk management, and technology cluster is evolving to create a more integrated and agile operating model, centred on core service offerings that position Medscheme as the foundation of AfroCentric’s healthcare solutions. This transformation enhances efficiency, service excellence, and cost sustainability while ensuring a seamless experience across all administered schemes. By refining how services are structured and delivered, the cluster is strengthening its ability to provide value-driven healthcare solutions while embedding a culture of innovation, collaboration, and continuous improvement.

A key focus has been improving workforce planning and recruitment to ensure the right skills are in place to support the evolving operating model. A more structured hiring process has reduced recruitment delays and improved talent acquisition, while workforce planning ensures staffing levels are aligned with service demand. This approach enhances resource allocation and supports a more resilient, high-performing team.

To strengthen clinical service delivery, the cluster has placed 65 physician assistants and case managers in KwaZulu-Natal, improving care coordination and expanding service capacity. At the same time, enhanced claims audit processes have reinforced financial oversight.

By aligning its operations with broader healthcare trends and ensuring that scheme administration remains cost-effective and patient-centred, the cluster continues to enhance value for clients, members, and stakeholders. These efforts contribute to a more cohesive and sustainable operating model that supports AfroCentric’s long-term strategic priorities.

Pharmaceutical cluster

AfroCentric’s pharmaceutical cluster has refined its operating model to strengthen integration within the Group, ensuring medicine affordability, supply chain efficiency, and service consistency. Scriptpharm’s integration has been a key focus to ensure cost-effectiveness and improved service delivery. Aligning its procurement and formulary strategies with AfroCentric’s broader healthcare objectives supports scheme sustainability and better patient access to medicines.
